Deposit & Payment
The online registration program will total your registration selections on the Payment Information page. As you input your payment information (credit card or bank account) you will see your full balance in the payment amount box. You have the option of changing this to the minimum deposit amount. No registration will be accepted without a deposit amount of $50 per week.
Discounts & Scholarship
All discounts will be applied to your account after your registration has been processed by the camp office. If you are interested in receiving a scholarship application, you must first register and then request an application from our business manager/registrar, Tyesha Smith (firstname.lastname@example.org, 847.740.5010 x 2225).

Prior to attending Camp, we need your help in getting all the necessary paperwork in by Thursday, May 15! As you finalize all paperwork, feel free to send it to email@example.com, via fax at (847) 740-5014 or by dropping it off at 26710 W. Nippersink Road.

All campers must fill out this form with their medical provider.
Camper Health History Form 2
This additional medication form is just for our Overnight Campers and must be signed by a doctor.
Medication Administration Form
If your child needs medication administered during their time at camp – whether it is an inhaler in case of emergencies or a regularly scheduled prescription medication – you must fill out this form to allow our camp nursing staff to administer that medication.
Authorized Pick-Up Form
Let us know who is allowed to pick your child up from day and overnight summer camp.
High Sierra Adventure Center Waiver
This must be signed in order for your camper to participate in the High Sierra Ropes Course!
We absolutely love referrals to Camp Henry Horner! To show our appreciation, we are delighted to offer a $50 refund for every new family you introduce Camp to. Please see our Family Referral Form for more information.
Every year we get asked about staff gratuities as a token of families’ appreciate for summer fun. It is our policy that individual staff members may not accept gratuities. It is our intention to create an environment that fosters teamwork and shared responsibility, and do not want to create a situation where one staff member receives something while others do not.
In order to recognize all of our staff in an equitable manner we have established a gratuity fund. If you are interested in contributing to this shared fund, please fill out our Staff Gratuity Form.
