We know your child’s preschool years are as much yours as they are your child’s – we do everything we can to build a sense of community and bring parents into the life of the center.
At JCYS Highland Park, family engagement is woven into the school year. We treat every family like our own and provide regular opportunities for parents to connect with each other and develop a community. Each year features multiple all-school events, including an ice-cream social to kick off the year, a Sukkah Party (complete with a petting zoo and glitter tattoo artist), and Hanukkah Party. Family events are scheduled in the evenings and on weekends, so parents don’t have to choose between work and school. We often hear families say they made their best friends at Lutz.
Below, you’ll find all the forms you may need for your child’s first day at JCYS Highland Park. All forms are required to attend JCYS as regulated by the Department of Child & Family Services.
Child Intake Form – Please complete even if you are a returning family
DCFS Certificate of Health Examination – Must be updated annually
Allergy Policy – Signature required for everyone
Allergy Emergency Action Plan – Required for any child with a life-threatening allergy or medical condition
Emergency Contacts – Required for new families. Returning families should fill this out if there have been any changes
Authorized Pick Up – Required for new families. Returning families should fill this out if there have been any changes
Late Pick-Up Policy – Signature required for everyone
Media Release Form – Required for new families. Returning families should fill this out if there have been any changes
DCFS Verification of Receipt – Required for new families
Challah Sale Form – If you want delicious challah to take home each Friday
Transportation Policy (required for K-5th grade) – Required for all K-5th graders
Waiver Agreement – Required for your child to participate in JCYS activities
Tuition Express – For automated tuition payment processing via bank account or credit card
Key Card Order Form – To grant parents access to the building
